As a Home Health Nurse, you will play an essential role in delivering patient-centered, high-
quality healthcare within clients’ residences. Core responsibilities include conducting health
assessments, developing individualized care plans, and administering nursing interventions to
support optimal patient outcomes.
Primary Duties and Responsibilities:
• Travel to patient homes to implement physician-directed care plans.
• Measure vital signs, including blood pressure, glucose, and administer medications and
insulin as prescribed.
• Assess and treat wounds, perform bandage changes, and support personal hygiene and
grooming needs.
• Evaluate muscle strength, identify pressure ulcers, and monitor for infection symptoms.
• Address concerns of family members by providing accurate information and support.
• Educate caregivers and family members regarding aftercare and ongoing patient needs.
• Communicate with physicians and families regarding patient status and recommendations
for improved care.
• Document patient progress comprehensively and prepare reports for medical review.
Qualifications:
• Bachelor’s degree in nursing or a related discipline required.
• Minimum of one year of experience in a supervisory capacity is required.
